Your knife is a Model #14 "Attack" with a stainless steel blade and sawteeth. The sawteeth are an option as is the stainless blade. The sheath looks like a standard "smooth button" Sullivan's Holster Shop with the correct stone. Both are correct for the knife.

Vintage is early to mid 90's. Could be later but that is a pretty popular knife so I would guess that the shop went through the smooth button sheaths pretty quickly after the change over to the logo button snaps you see on today's sheaths.
